Social Services
Social Services works to meet patient needs for economic, social, physical, and ministerial services.
- Coordinates discharge planning process including arranging adaptive equipment, home health and other care facility services and transportation needs.
- Maintains knowledge of community services, agencies, and resources available to patients.
- Assists in completing advanced directives.
- Interacts with patients, families and service organizations to promote continuity of care.
